Welcome Back Sailfish Family!

As the sun sets on another summer, I would like to welcome you back to campus for what is sure to be another successful season here at Palm Beach Atlantic University in 2016-17.

The information in this letter is important as you prepare to make your way to beautiful West Palm Beach. Please take the time to read this letter in its entirety. Failure to submit requested information may result in you being ineligible for practice and/or competition.

Updated Physical

You must submit your physical directly to Sam Eisen, Head Athletic Trainer, at sam_eisen@pba.edu or Michelle Menard, Assistant Athletic Trainer, at michelle_menard@pba.edu. As a student-athlete, we can only accept a physical that was completed not longer than 6 months ago and administered by an MD, DO, Nurse Practitioner or Physician Assistant (if co-signed by the Physician). If you sustained an injury requiring you to see a physician within the last 12 months, you must also provide a letter from that physician stating your diagnosis and participation status.

Registration

All Palm Beach Atlantic University student-athletes must be enrolled full-time. In order to be considered a full-time student, undergraduate students must be registered for 12 credit hours and graduate students must be registered for 6 credit hours.

NCAA Athletics Participation Forms

You were sent an email from Goeun Lee, Assistant Athletic Director for Compliance, related to setting up an account and completing required NCAA forms through our online compliance system, Jump Forward. Please complete all 13 forms not later than Friday, August 5, 2016. If you are under the age of 18, your parent or legal guardian must co-sign all of the forms. If you have any questions related to Jump Forward, contact Goeun directly at goeun_lee@pba.edu.

Proof of Insurance

You are required to provide proof of insurance to Sam Eisen, Head Athletic Trainer, at sam_eisen@pba.eduor Michelle Menard, Assistant Athletic Trainer, at michelle_menard@pba.edu. Please verify with your insurance company that you are covered for athletic injuries and that you are covered to see a specialist or to get an MRI. If your insurance company does not have a United States address and/or phone number, you will need to inform the Athletic Training Staff so you can purchase the NCAA Primary Policy to cover your athletic related injuries. The cost for the NCAA Primary Policy ranges from $475-$775. Please note that PBA provides secondary insurance that covers student-athletes for athletically related injuries.

Online Check-In

Each term, online check-in is available for all registered PBA students. By using the online system, you will be able to address most issues before arriving on campus (i.e. view pending requirements and/or holds and activate your PBA card). Visit My.PBA.edu to access the online check-in system.

Athletic Training

Student-athletes and parents must acknowledge that there is an inherent risk of injury during intercollegiate athletic participation. PBA provides a medical staff of certified athletic trainers and team physicians for prevention, management, and care for our student-athletes. For liability and insurance purposes, you are required to see our team physicians first before seeing any other doctor for an athletic injury. Ultimately, our team physicians will have the final say on student-athletes returning to practice and/or competition. Services that are rendered in the PBA Athletic Training Room will be billed to your primary insurance. Please note that there will be no additional out of pocket costs to you and your premiums will not go up.

Insurance Billing

If you are injured while participating in a PBA sponsored athletic activity, you will be covered by the department’s athletic insurance as long as institutional protocol was followed. Anything that is denied coverage by your primary insurance carrier will be paid by PBA. Should you follow the PBA injury protocol, you should have no out of pocket expense for medical care received for an athletic injury that occurred while representing the University. All medical bills should be submitted to the Athletic Training Staff. DO NOT pay for any medical bills received.

Welcome to the Sailfish Family!

We are excited that you will be joining us soon and we hope that you are able to attend all of the 2016 Welcome Week activities Thursday, August 19 through Tuesday, August 23, 2016. Please note that the Athletics Department is hosting a New Varsity Student-Athlete Parent Meeting on Friday, August 20, 2016 from 3 to 4 p.m. in the Rubin Arena. We strongly encourage your parents to attend. Entering college is an exciting time but it can also be an unsettling transition with all of the unknowns and new experiences ahead of you. Welcome Week is a perfect opportunity to ease into this transition before the start of classes. We believe you will find some comfort in the supportive and uplifting community of your fellow students, faculty and staff who will be important contributors to your collegiate experience.

Final official transcripts from your previous institutions (i.e. high school, junior college or four-year university) must be submitted to the Office of Admissions as soon as possible.
